MINOCQUA DANCE, LLC POLICIES

Please initial next to each policy showing that you understand and will adhere to the policies set forth by Minocqua Dance, LLC.

REGISTRATION

_____Classes are subject to cancellation, pending enrollment. To reserve your spot in that class, registration & 1st payment is due by Friday, September 1, 2023.

_____ If you register and take a space in a class, you are committed to and agree to pay tuition for that class until the end of the season, May 2024 unless you utilize the Drop Out Deadline; Friday, October 27, 2023.


TUITION

_____ I understand that Minocqua Dance tuition is nonrefundable. No credit or refund is given for missed classes or if we need to switch to ZOOM virtual classes. I only have until Friday, October 27 to drop out; otherwise I’m obligated for the 2023-2024 season. Tuition is for lessons only. Additional fees for performance and costume/costume cleaning.

_____Payments are due by September 1, October 26, January 11, & March 7

_____All outstanding balances must be in full before (i) costumes are handed out, (ii) any student can perform in the recital, (iii), and/or registering for any classes. All unpaid charges after the payment dates are subject to a $15 late fee.

_____Accounts with a history of non-payment or non-compliance with studio policies may be barred from registration and may be asked to leave the studio or may be asked to prepay tuition.


CLASSROOM RULES

_____ Attendance is imperative for consistent progress. Each student is expected to attend the entire dance season.  Attending the last two classes before a performance, and the dress rehearsal, is mandatory. It is my responsibility to communicate with Minocqua Dance staff via email when my dancer is going to miss class/rehearsal and to give a 60-day written notice for any missed performances so that instructors can re-do choreography and partner work so that all dancers can perform with confidence.

______ Dancer(s) must follow the Minocqua Dance dress code including hair, shoes, and clothing.

______Label all dancewear, shoes, and accessories. Minocqua Dance is not responsible for lost property.

­­______In the event we need to switch to ZOOM virtual classes, class times will remain the same & pointe work will be limited.


STUDIO COMMUNICATION

______Minocqua Dance communicates primarily through email based on the email address you provide the studio. Please note if you UNSUBSCRIBE to our email service, we cannot add you back on our email list and it is your responsibility to re-subscribe. 

______All communication about classes, enrollment, policies, or billing must be communicated through the Minocqua Dance email or phone.


OTHER POLICIES

­______We have allowed for one Snow Day per class if classes are canceled due to weather. 

______With prior instructor approval, students can make up a missed class at a level down or at level. 

­______No eating or drinking in building except water.

______Minocqua Dance reserves the right to take and use photography of Minocqua Dance students for advertising and promoting the studio. It is my responsibility as the parent/guardian to inform Minocqua Dance in writing if I refuse permission. 

______Please be advised that the study of dance involves physical contact. Instructors may need to make hands-on contact to adjust students’ alignment and to demonstrate proper motion. 

______It is the parent/guardian’s responsibility to keep informed of all policies including safety, requirements, and deadlines. All parents and students are expected to read and comply with all studio policies and procedures, newsletters, recital information, etc. available via e-mail, on the website, and at the studio.

WAIVER OF LIABILITY

I have received, read, and will adhere to the policy & safety procedure statements of Minocqua Dance, LLC.

In consideration for the dance instruction and activities to be received from Minocqua Dance, LLC

I agree to the following:

1.     I understand and acknowledge that the dance instruction and activities in which I am about to voluntarily engage, bear certain known risks and unknown risks, which could result in personal injury to myself or through my actions, to others.

2.     I represent that I have no health or physical condition that would prevent me from engaging in the proposed dance activities or increase the risk of injury to myself, or others.

3.     I understand that by signing this agreement I will have no right to make a claim or file a lawsuit against Dana Bostrom, dba Minocqua Dance, LLC or its agents, employees and representatives for injuries or damages incurred by me even if caused by the negligence of Minocqua Dance, LLC. My signature indicates that I have read this entire document, understand it completely, and voluntarily sign it.
